Adelaide, Australia, 12 August 2026 – A South Australian coronial inquest has found that the 2021 death of 32-year-old school librarian Kate Sylvia was preventable, identifying two missed opportunities to send her to hospital after serious neurological symptoms were assessed as a migraine.

Sylvia developed vomiting, stabbing head pain and slurred speech before she died from cerebral venous sinus thrombosis, a condition involving a clot in the brain’s venous drainage system. Four days before her death, an ambulance took her to an intermediate-care centre at Sefton Park rather than a tertiary hospital.
